Tender description

NHS Southern Eastern Hampshire and Fareham and Gosport CCGs invite Expressions of Interest from all suitably qualified and experienced healthcare providers (including general practitioners, other clinical providers, social enterprises, third sector organisations and other providers) to provide Alternative Provider Medical Services within the Havant locality (currently operating out of the Havant Health Centre under a GMS contract), led by the NHS Commissioning Board. Key Objectives of this Procurement: a) To deliver the procured services in full on the 1.4.2018 to a patient list of 8 200; b) Length of Contract will be a minimum of 3 years but may be tendered for up to 5 years; c) To ensure provision of a skilled surgery workforce delivering primary care to the registered population within a defined resource; d) To ensure delivery of a responsive service addressing the needs of the patients and their carers, as per the requirements of a GP surgery providing a GMS contract; e) To maintain and exceed the current achievement level of the National Quality and Outcomes Framework and to work closely with the local CCG and other GP Practices to ensure local targets and standards are met; f) To deliver affordable and value for money (VfM) medical services; g) To provide convenient, dependable and easily accessible services; h) To provide services that are operationally stable, clinically safe, patient-centred and cost effective; i) To prevent significant deterioration, particularly in vulnerable groups, through effective chronic disease management. Download the attached EOI Registration document, complete and then upload to the portal via the project messaging service.
