Tender description
The North West London Delivery Area 3 (Older people’s care) programme board and Joint Health and Care Transformation Group (JHCTG) have agreed an ambition to strengthen and go further on borough (CCG and Council) level joint integrated commissioning of community services. Initially the focus will be upon care homes and Continuing Healthcare as a minimum, as a critical enabler towards accountable care. Each North West London borough will submit a draft plan to the Health & Well Being Board outlining its scope and future intentions.
