Tender description

Primary Care general practitioner services for the re-directing of most minor illness appointments away from GP practices to primary care hubs in each locality of the CCG so that more time can be spent by GPs with their patients who have a long term condition. Primary care access hubs available only for registered list patients of GP surgeries who are both members of NHS South Kent Coast CCG and registered with a GP practice in the locality of the specified hub in which the GPs from those practices will work. The services will be led by doctors from the locality practices alongside multi-professional teams. The days of operation will be 8:00-20:00 daily. The effect will be to have a branch for GP practices, that deals with minor illness appointments.
