Tender description

Arden and Greater East Midlands Commissioning Support Unit (Arden & GEM CSU), acting on behalf of Nottingham City CCG (themselves acting under co-commissioning delegated authority from NHS England) seeks a provider to offer high quality and safe GP services (in accordance with General Medical Service Regulations) for circa 3,179 raw/3,639 weighted patients from the existing purpose built premises located at 17a Tremayne Rd, Nottingham NG8 4HQ, from 1 October 2016. Nottingham City CCG (the Commissioner) will be the contracting and commissioning authority for the contract resulting from this procurement. The practice boundary for patient registration extends to a three mile radius from the practice towards the City centre, and includes a large portion of the western side of the Nottingham City local authority area. There are six other practices within one mile of Grange Farm. The attached Care Delivery Group Health Profiles give details for both Grange Farm Medical Centre and those practices nearby. There is unlikely to be significant housing growth in the vicinity of the Medical Centre but there is potential to increase the list size. Grange Farm Medical Centre is one of 57 GP practices providing primary medical services within Nottingham City CCG area. The main local acute hospital is Nottingham University Hospital NHS Trust; community services and mental health services are provided by the Nottinghamshire Healthcare NHS Foundation Trust. For this service provision Arden and GEM CSU would like to encourage innovative solutions including consortia, strategic alliances and sub-contracts which will deliver value for money and improved services to patients. The services must be provided by GPs and Nurses and they will be required to deliver an outcome based service to include: — High quality Primary Medical Services responsive to the needs of the local population; — Provision of professional advice to patients and promotion of healthy living with the aim of tackling the underlying causes of ill health; — Provision of all essential and additional services; — All Enhanced Services commissioned by NHS England, the Clinical Commissioning Group (CCG) and the local authority; — Robust business management systems and processes for improved patient experience; — Financial management — efficient use and maximisation of available resources. Maintain financial control within the available budget; The successful provider will need to demonstrate that they can deliver service(s) of a continuously-high quality, ensure a sustainable workforce and be responsive to the needs of the local population. The providers will be required to establish and maintain best practices and use them for continuous service improvement during the term of the contract. The providers will also need to adopt applicable national guidance. The Commissioner encourages innovation and use of technology within service delivery. It is seeking patient centred solutions for the provision of service that consistently delivers high levels of patient satisfaction with the following aims: — The increased and right access — the method of service delivery needs to be flexible to meet the varying patient needs; — The right clinical assessment — specialist assessment for children and elderly patients or that with a disability; — The right timing — the timely inspection and screening of vulnerable patients for prevention of any complications. Contract value: The contract value in year 1 will be based on 108 GBP per weighted population; there will be annual reductions in the GBP per weighted population over years 2, 3 and 4 and from year 5 onwards the contract value will be based on the global sum at that time per weighted population. Contract duration: The current APMS contract ends on 30.9.2016 and the Commissioners will be seeking to re-procure services under the terms of an APMS contract for 10 years plus 5 years possible extension subject to annual reviews. Type of procurement procedure: The procurement will be conducted as an OJEU (Part B) Service Contract using the Open Procedure under the Public Contracts Regulations 2006. Award criteria: The most economically advantageous tender derived using the criteria stated in the specifications of the Invitation to Tender (ITT).
