Tender description

NHS Arden and Greater East Midlands Commissioning Support Unit (Arden and GEM CSU) on behalf of Southern Derbyshire Clinical Commissioning Group (SDCCG) and NHS England (North Midlands) has carried out an exercise to identify a caretaker provider for a GP Surgery on a short term APMS Contract. Southern Derbyshire Clinical Commissioning Group (SDCCG) and NHS England (North Midlands) were looking for a suitable qualified provider with the ability to deliver a GP Surgery service for up to 12 months (9 months plus 3 month notice period). During the 12 month care taking arrangement, the long term future of the service will be determined through public consultation and, depending on the outcome of the consultation, possible re-procurement. The practice has a current list size of approximately 11 000 patients and is provided from 2 locations: The Meadowfields Practice — Chellaston. Fellow Lands Way, Chellaston, Derby, DE73 6SW. The Meadowfields Practice — Allenton. Coleman Health Centre, 6 / 13 Coleman Street, Allenton, DE24 8NH. The identified provider will have the option to continue to provide services from these premises. The Commissioners are looking for a modern, patient-centred model that is user friendly and culturally sensitive that considers all of the national and local public health targets. The model will need to include provision of the following: — Essential Services; — All Additional Services; — Dementia and Learning Disabilities; — Enhanced Services. — Opening hours and service delivery from 8:00 to 18:30, Monday to Friday (except statutory bank and public holidays). It was essential that the provider was able to demonstrate an ability to mobilise to service provision within a short time frame. The Care taking Service Commences 1.1.2016.
