Tender description
The London Borough of Hammersmith and Fulham acting on behalf of itself and the Royal Borough of Kensington and Chelsea and Westminster City Council invites Tenders for the provision of 3 Carers' Hub Services. 1 Carers' Hub Service will operate in the London Borough of Hammersmith and Fulham, 1 Carers' Hub Service will operate in the Royal Borough of Kensington and Chelsea and 1 Carers' Hub Service will operate in Westminster City Council. Each local authority will contract directly with the provider of the Carers' Hub Service which will be selected to operate in their own area. It is intended each Contract will have an initial term of 3 years The contracting local authority will be entitled at its absolute discretion to extend the Contract Period on the same terms for 2 further periods each of 1 year making a total possible Contract Period of 5 years. Each council and its respective Clinical Commissioning Groups(s) is committed to increasing the health and well-being of the population and enabling people to directly manage their own care, maximising choice, independence and control. The commissioning of a Carers' Hub Service to operate in each borough is seen is key in contributing to an improvement in the health and well-being of carers and to strengthening and sustaining them in their caring roles. The service will support each Council to enact their new duties under the Care Act 2014. The Councils consider the services are most effectively provided by using a Hub model approach which will co-ordinate support for adult Carers within each borough, providing 1 point of contact for Carers to improve access to all relevant local support and community assets. The procurement is designed to ensure that successful tenderers are able to deliver a comprehensive support service for adult carers over the age of 18 and young carers who are transitioning from Young Carers Support Services as they approach the age of 18.
