Tender description

We are seeking product information and a system demonstration from organisations that can provide:- a) A software package that delivers an end of life (EOL) register. It must contain essential patient details,preferences for their palliative care and be easily accessible,extract data,provide read only access and updatable by the patient and the professionals involved in their care. b) A platform for an Electronic Palliative Care Co-ordination System to enable the sharing of the end of life (EoL) register by care professionals. It must be able to show by individual,a single shared care plan,providing a consolidated view of care packages from disparate care settings and source systems;pulling information from core clinical and care IT systems &/or databases across the whole health system Options A and B may/may not be procured/supplied separately but must be compatible. NHS Kernow wishes to explore what products are available in the market to fulfil the above scope. NHS Kernow may wish to scale out any potential project to include NHS Devon. Funding has yet to be identified. We would like to invite organisations that can supply a hosted or non-hosted system to provide information about their products and demonstrate their systems early Dec 2016 which will help inform a service specification.
