Transitional Support & Resettlement Service in a Residential Care Setting

Tender description

The Transitional Support & Resettlement Service is bed-based in the community, providing people with a place to rest and recuperate whilst social services arrange onwards transfer of care. The Service supports capacity for local authority social workers to make arrangements for both avoidance of an inappropriate admission to hospital and/or a permanent care home or for the transfer of care from hospital whilst further work is undertaken to complete arrangements for assessment of long-term care and support. The Service will be delivered through a residential care setting (a care home).
