Tender description
NHS England South (South West) wishes to identify provider(s) for GP Services in Plymouth. NHS England South (South West) is looking for applications from providers for the following lots. Lot 1 Ernesettle Primary Care Centre, (Raw List 4317). Lot 2 Mount Gould Primary Care Centre (Raw List 2411). Lot 3 Trelawny Surgery (Raw List 2660). Lot 4 Barne Barton Surgery (new service). NHS England South (South West) proposes to use either the NHS General Medical Services (GMS), NHS Personal Medical Services (PMS) or NHS Alternative Providers of Medical Services (APMS) contract for these services, depending on the type of contract held by the successful bidder. This will be by way of variation of the existing contract for GMS/PMS Lots 1, 2 and 3 are currently one practice with a list size of 9388 over 3 premises leased from landlords who have indicated that they would be available for future providers. This catchment area covers the whole of Plymouth The table below shows the practice profiles for both practices compared to the CCG average Lots 1,2 and 3 as a whole New Devon CCG average Patients in Nursing Homes 0.66 % 1 % BME Patients 8 % 3 % Patients on disability living allowance 9 % 5 % Deprivation ( IMD) 35.43 20.49 Lot 4 Barne Barton does not have premises. NHS England South (South West) wishes to commission a service to be delivered from a new purpose built primary care facility on Miers Close next to the Tamar View Community Centre. Barne Barton has a growing population currently around 5,000. Demographically there are a much higher proportion of young people than in Plymouth as a whole: — It has the highest percentage of under 5s in the City (10.7 % compared to the City average of 5.7 %), and also of children aged 5 to 14 years (16.1 % compared to an average of 10.2 %). It is known that families with children under 5 are amongst the highest users of primary care services. — It has the lowest percentage of over 75s (2 %) compared to the City average (7.4 %). The successful provider(s) will need to provide the full range of primary medical services from 1.4.2017 for Lots 1-3 and on opening of the new purpose built primary care facility for Lot 4 Services to be provided NHS England is looking for an innovative and patient-centred model that is user friendly and culturally sensitive and encompasses national and local public health targets. The model will include provision of the following: — Essential Services. — All Additional Services. — Enhanced Services. — Core opening hours and service delivery from 8:00 — 18:30 Monday to Friday (except statutory bank and public holidays). The full scope of the service can be seen in the service specification. It is essential that interested providers can demonstrate an ability to mobilise to service provision.
