Tender description
The London Borough of Bexley is undertaking a procurement process to identify organisations that are able to deliver an outreach health check service to supplement its GP health check service throughout the borough by engaging with those difficult to reach communities to encourage uptake of the programme and in particular to increase uptake amongst relatively deprived and hard-to-reach groups. The provider will deliver tailored face-to-face feedback for each individual on their future risk of cardiovascular disease. In addition to the appropriate medical management of risk this will include advice on lifestyle and referral to local lifestyle interventions as appropriate. Keywords: Healthchecks
