Nottinghamshire County Council requires an organisation to provide and implement an NHS Health Check IT Solution tool for GP practices, compatible with GP clinical systems i.e. SystmOne, EMIS LV, EMIS Web, EMIS PCS, plus others not listed here over N3, which will identify the eligible population and monitor individual's status to NHS Health Check completion and a reporting mechanism to provide anonymised reports to aid performance management and national reporting requirements and compliance.
