Chichester Wellbeing is looking to commission sustainable activities that support behaviour change in those with increased risk of developing cardiovascular disease and cancer, as part of the countywide Prevention and Wellbeing Programme. These activities will be evaluated as part of the wellbeing programme. Chichester Wellbeing works to tackle the wider determinants of health; tackling factors which affect health and wellbeing and health inequalities. Aim: To provide a community based falls prevention programme for older adults at risk of falling and to provide a step down service to prevent further falls for those who have completed the NHS falls and fracture prevention service.
