Falls Prevention Advisory Service and Strength and Balance Programme

Bracknell Forest Public Health is looking to commission one provider to provide a Falls Prevention Advisory Service The service will be available to all Bracknell Forest residents aged 65 and over. The adviser(s) will be trained professionals able to carry out a detailed assessment of falls risk within the home with a view to designing (in collaboration with individuals and families) a package of community based support and advice. The service will be beneficial for any individual who is assessed as being at a ‘medium risk’ of falling as defined by the outcome of a standardised screening tool (based on existing accredited risk assessment tools). The service aims to prevent falls before they occur. Individuals may self-refer or be referred by a professional or agency. A Strength & Balance Service This service is a rolling 12 week programme of strength and balance classes that are available to Bracknell Forest residents aged 65 and over. In order for the Council to keep organisations updated with any clarifications or changes to documents, Organisations must register their interest against this advert. The Council will not be held responsible for not communicating important information to any organisation who has not officially registered their interest.
