Tender description

NHS Arden and Greater East Midlands Commissioning Support Unit (Arden & GEM CSU) on behalf of Southern Derbyshire Clinical Commissioning Group (SDCCG) and NHS England (North Midlands) is carrying out an exercise to identify a caretaker provider for Meadowfields GP Surgery on a short term APMS Contract. Southern Derbyshire Clinical Commissioning Group (SDCCG) and NHS England (North Midlands) are looking for suitable qualified providers with an ability to deliver a GP Surgery service for up to 12 months (9 months plus 3 month notice period). During the 12 month caretaking arrangement, it is envisaged that the long term future of the service will be determined through public consultation and, depending on the outcome of the consultation, possible re-procurement. The practice has a current list size of approx. 11,000 patients and is provided from two locations: The Meadowfields Practice - Chellaston Fellow Lands Way, Chellaston, Derby DE73 6SW The Meadowfields Practice - Allenton Coleman Health Centre, 6 / 13 Coleman Street, Allenton, DE24 8NH The identified provider will have the option to continue to provide services from this premise. The Commissioners are looking for a modern, patient-centred model that is user friendly and culturally sensitive that considers all of the national and local public health targets. The model will need to include provision of the following: • Essential Services • All Additional Services • Dementia & Learning Disabilities • Enhanced Services • Opening hours and service delivery from 8am – 6.30pm Monday to Friday (except statutory bank and public holidays). The full scope of the service can be found in the attached draft specification; it is essential that interested providers must be able to demonstrate an ability to mobilise to service provision within a short time frame. The Caretaking Service is due to Commence 1st January 2016. The Commissioners wish to invite suitably qualified and experienced providers to bid for the provision of this service. Commissioners are asking that all providers are able to answer ‘yes’ to the following questions before bidding for this opportunity: 1. Is your organisation CQC registered? 2. Do you have 3 years of audited Accounts? 3. Do you have staffing capacity available to deliver this service for 1st January 2016? 4. Have you delivered a caretaking or similar role in the past? 5. Are you able to ensure the service will commence from 1st January 2016?
