Provision of Primary Medical GP Services at Middleport Surgery, Newport Lane, Stoke-on-Trent, Staffordshire ST6 3NP

NHS Arden and Greater East Midlands CSU (NHS Arden & GEMCSU) on behalf of NHS England North Midlands (referred to as the Commissioner) is inviting suitably qualified and experienced providers to deliver primary General Medical Services through an APMS contract only at Middleport Surgery, Stoke on Trent, Staffordshire ST6 3NP. Middleport Medical Centre a GP-led health centre was commissioned by Stoke on Trent Primary Care Trust (PCT) in 2008 as part of the national improving access in primary care programme led by Lord Darzi. The current APMS contract has previously been extended and now expires on 31st March 2016. The key objective of this procurement is to commission an able provider through an APMS contract for the provision of primary medical services Middleport. That meets the technical and resource capability for the commissioning organisation; NHS England North Midlands. The provider will be assessed on an ongoing basis to ensure they deliver the quality required. The services must be provided by GPs and nurses and the provider will be expected to deliver: a) High quality primary medical services responsive to the needs of the local population, b) Promotion of healthy living with the aim of tackling the underlying causes of ill health, c) Essential services, d) All additional services, e) All enhanced services commissioned by NHS England, the clinical commissioning group (CCG) and the local council, f) Increased access, g) Proactive model of care for nursing and residential homes including Scotia Heights which provides residential care for both permanent and respite residents who are young adults and have physical disabilities and mental health problems. This alternative provider medical services (APMS) contract has an anticipated start date of the 1st April 2016 for a period of 10 years (with a break clause after 5 years for either terminate the contract should the so wish giving 6 months notice) with the option to extend for a further 5 years. The maximum contract duration will be 15 years in total including any extensions. The Practice resides in a LIFT build and leases the premises at Newport Lane from CHP who will be able to provide details of the leasehold arrangements. The New provider must have signed the lease agreement with the landlord at least 1 month prior to the Service commencement date. This APMS contract will be subject to all relevant Regulations, Directions and Legislation. Staff will be subject to TUPE arrangements please see Supporting Information provided in the Attachments section on Bravo. The ITT Reference number is 'itt_309'. Contract value excluding VAT is £540,280 per annum, with a total contract value excluding VAT for the contract duration of 10 years being £5,402,800. There are further enhanced services in the APMS Contract that may inflate this figure Anticipated timeline milestones are:- 1. Contract Award - End December 2015 2.
