Tender description

NHS North East Essex are currently seeking feedback and engagement for suppliers able to offer Stroke Rehabilitation services for Early Supported Discharge. The Early Supported Discharge Team for stroke survivors will provide an early, seamless transition from hospital to home, through a multidisciplinary, co-ordinated specialist stroke rehabilitation team. Providing therapy, advice and support to people who have experienced a stroke, including their family /carer. Integrating Health, Social and voluntary organisations to ensure a high quality pathway from transfer from the acute setting through discharge home and beyond. The service will be available to those people who have experienced a new stroke, currently within an acute setting (or previously known to the service) and are registered with a GP within North East Essex
