Home Not Hospital: VCS Support for Admission Avoidance, Discharge, and Recovery

Tender description

The Authority is inviting bids from the voluntary and community sector (VCS) to deliver support services in North East Lincolnshire aimed at hospital admission avoidance, discharge facilitation, and recovery support. The Service focuses on reducing hospital admissions and promoting independence through two initiatives: Enablement Champions and Home from Hospital/Reablement. The Authority has determined, prior to inviting tenders, that participation in this procurement is restricted to providers based within the defined local area of North East Lincolnshire. Accordingly, providers not based within this local area (being those without substantive business operations in the area, as defined by the Order) will not be permitted to participate. Participation in this procurement is also restricted to Voluntary, Community and Social Enterprises (VCSE). Accordingly, providers that are not VCSE’s will not be permitted to participate
