Tender description

The aim of the service is to provide safe, effective support for carers during acute hospital admissions (physical and mental health) of the individual for which they provide care, improving outcomes, experience, and resilience—helping people remain in their chosen home for longer, reducing avoidable reliance on statutory services, and delaying/preventing long-term care admission. Our focus is on what matters most to carers—delivering the greatest impact on their lives. By doing this, we aim to achieve better experiences and improved quality of life for carers on the Isle of Wight.
