NHSE Health and Growth funded project piloting a new approach for caring for people with multiple LTCs. The provider will adopt a new way of working to improve and understand how we can best configure services around the needs of people with multiple LTC's (with a focus on the working age population from September within the cohort, still focusing on our priority PCN areas). Lot 1: This notice is confirmation of an award having been made due to an urgent circumstance. The Contract Authority; NHS West Yorkshire Integrated Care Board has awarded a contract under the PSR Urgent Award process.
