Tender description

Reablement support is a therapy-led short-term service that helps people regain their independence, often after an illness or hospital stay. Reablement focuses on achieving goals and helping someone to regain skills and confidence. This service differs from domiciliary care as its aims are beyond ‘maintaining’ people to live independently in their own homes. This will be an activity based contract with payment linked to the volume of service delivered up to the maximum allocation set out in this report.
