Tender description
North Northamptonshire Council (NNC), in partnership with Northamptonshire Integrated Care Board (ICB), is seeking to establish a Framework Agreement for the provision of Supported Living Services for Working Age Adults. <br /><br />This service is for adults aged 18 and over with a Learning Disability and/or Autism, who have been assessed as having eligible needs under the Care Act 2014. The core aim is to enable Individuals to live as independently as possible in their own homes within the community, with care and support that is flexible, responsive, and focused on achieving personal outcomes.<br /><br />The support offered under this Framework is designed to be a long-term arrangement, promoting skill development, community inclusion, and maximising independence. Providers will deliver outcome-focused support that empowers Individuals to maintain their tenancy, exercise choice and control, and live a fulfilling life free from harm.<br /><br />The Services covered by this Service Specification are for Individuals who have a Learning Disability, Mental Health Needs and/or who are Autistic, and who have been assessed as having eligible needs under the Care Act. The Council act as the lead commissioner for these services, in workforce with Northamptonshire Integrated Care Board (ICB). <br /><br />The following Lots detail the Services to be initially procured:<br /><br />• Lot One - Individuals with Learning Disability<br />• Lot Two - Individuals with Mental Health Needs<br />• Lot Three - Individuals who are Autistic <br /><br />PLEASE NOTE <br />Providers that are interested in providing services to the Call off from the Framework Agreement for Supported Living Services for Working Age Adults must complete and submit the appropriate LOT Questionnaire on Intend Reference NNC00000697 at the same time as they apply to the NNC in Partnership with ICB for the Provision Of Supported Living Services For Working Age Adults Framework. <br /><br />In order to be considered for this opportunity, you MUST have registered and submitted your core supplier information to the Central Digital Platform (CDP).
