Tender description

The Supplier will be contracted to provide Social prescribing to children and young people aged 11-25 yrs to help improve health and wellbeing. The supplier will provide experienced link workers who will support each young person to create a personalised plan and set goals based on what is important to them. :<br/>EG To access support services<br/>To access hobbies, activities and voluntary opportunities<br/>To learn techniques to support their wellbeing whilst waiting for specialist support<br/>They offer 1-1 and group health and wellbeing sessions<br/> where in-person social prescribing assessments are offered. This contract will be for one year with the option of extending by a further year Lot 1: the Trust intends to award to an existing provider following direct award c<br/>This contract will run from July 2025 for one year with an option to extend for a maximum of 1 year<br/>The lifetime value of the contract including options is approximately £39974.40
