Social, Personal & Nursing Care for Adults within a Specialist Hospital Setting (Complex Care) in Liverpool, Knowsley, Warrington and Halton

Services provided within a specialist hospital setting, able to meet the assessed social, personal, nursing and healthcare needs of an individual, such needs being detailed within an agreed Care Plan for each Individual Service User Placement and met by a full multidisciplinary team (MDT approach). Lot 1: NHS Cheshire and Merseyside ICB has awarded a contract via Direct Award B to deliver a complex placement (Mental Health and Learning Disabilities) service across Cheshire & Merseyside. The contract will start on the 01/04/2024 to 31/03/2025. The contract will be of nil value and will include individual place agreements within the contract that will include agreed costs. These are existing providers delivering existing services for Cheshire & Merseyside Liverpool, Knowsley, Warrington and Halton patients.
