North Northamptonshire Council - Provision of Targeted NHS Health Checks Community Programme

North Northamptonshire Council has agreed £28,000 extra funding to enable providers to deliver 560 targeted health checks (THC) in community settings over three months, with the flexibility of a further extension. The programme will gather evidence on the feasibility and impact of conducting THCs. The THC programme will provide an intervention to support and offer advice to target populations to help prevent the onset of CVD and help identify their risk of contracting CHD, stroke, kidney disease and diabetes. The intervention will be given to eligible people who are not currently on a CVD register or being treated as at-risk and are aged between 40 and 74. The objective is to support individuals from target populations in managing and reducing behavioural risks and associated conditions through information and an evidence-based risk reduction intervention. Lot 1: North Northamptonshire Council (NNC) (hereafter referred to as "The Council") invites quotations for the provision of the call off for community targeted NHS health checks under North Northamptonshire council - NHS health checks dynamic purchasing system (DPS). North Northamptonshire commissions the NHS Health Check (HC) service. This is a free check-up of your heart and blood vessels (cardiovascular health). These checks can identify whether someone is at a higher risk of contracting preventable cardiovascular disease (CVD) or its associated diseases, such as coronary heart disease (CHD), diabetes, kidney disease and stroke. Only providers who are on the NORTH NORTHAMPTONSHIRE COUNCIL - NHS HEALTH CHECKS DYNAMIC PURCHASING SYSTEM (DPS) NNC00000533 are eligible to apply for this Call-Off. Potential providers who apply to this Call Off and are NOT on the NHS Health Checks DPS will not be considered for this Call Off and will be excluded from the process.
