Medical Practice and related services<br/>Pan-Camden outreach bus, focussed on the most deprived wards, with the aim to reduce the barrier of access to healthcare for people in these areas by partnering with Camden council. The Outreach bus collects info on: smoking status, BMI, blood pressure, Know Your Risk score. Clinicians onboard have conversations around diabetes/pre-diabetes risk, whether they are registered with a GP (and encouragement to do so), as well as providing onward follow ups with GP for diabetes/hypertension/NHS check, other health interventions and sign posting towards council and VCS supported H&WB initiatives. Clinicians onboard have conversations around diabetes/pre-diabetes risk, whether they are registered with a GP (and encouragement to do so), as well as providing onward follow ups with GP for diabetes/hypertension/NHS check, other health interventions and sign posting towards council and VCS supported H&WB initiatives.<br/>This notice is an intention to award a contract to the existing provider following direct award process C.<br/>The lifetime value of the contract is £64,800. <br/>The expiring contract ends 31/03/2025<br/>The contract term will be 1 year with no option to extend.<br/>The contract will start on 01/04/2025 and end 31/03/2026.
