Family Hub: Family hubs work including administration tasks, outreach work and organisational support to Family hubs coordinator Team Around the School Pilot (TAS) Case Coordination: • Working with schools to ensure meeting papers are ready and circulated on time. • Capturing decisions and actions in meetings in real time. • Summarising key actions from previous meetings, track progress on agreed actions, and ensure that all necessary services are coordinated in an effective and timely manner. • Recording meeting metrics Offer updates on communication between involved system partners/services and identify any delays or arising challenges in service access or assessment
