Tender description
This service is intended to support any resident of Bracknell Forest, aged 18+, requiring additional support on discharge from hospital. The service will liaise with key stakeholders to receive and action referrals. People will be supported for a short period of time with both visits and telephone calls to support emotional needs, small practical tasks and relevant signposting and guidance.
