Bedford Borough Hypertension Management Service - BEDOC (Bedford on Call)

Primary Aim: The primary aim of the service is to provide an equitable, effective and efficient centralised care pathway, within available resources to improve access to hypertension reviews and to improve the blood pressure management of the Bedford Borough (BB) population with diagnosed hypertension, reduce health inequalities by prioritising those with the most unmet need in hypertension management and ultimately reduce cardiovascular outcomes. Objectives: This is expected to result in: • Greater numbers of people with hypertension in BB treated to their age-appropriate BP target. • Improve treatment to target for people with hypertension living in the most deprived areas and those with the greatest need, improving overall performance and reducing inequalities in hypertension management and outcomes. • Have a lasting impact to improve and streamline approaches to hypertension management across BB in line with the current work across BLMK, including support for self-management. • Support greater awareness in the population of hypertension and the importance of managing blood pressure to reduce cardiovascular risk. • Reduce CVD events across BB in the long-term.
