Digital health Hub for Patients and Carers with Rare Diseases

Tender description

The NHS Wales Shared Services Partnership, hosted by Velindre NHS Trust are acting on behalf of Cardiff and Vale University Health Board (UHB) and NHS Executive who are seeking to establish a five year contract to develop a digital health hub for patients suffering from rare diseases, and their carers, who are registered in Wales. The digital health hub will be a regional approach for AB UHB, C&V UHB and CTM UHB. Lot 1: Care Circle will work with Cardiff and Vale UHB and the rare disease community to develop a digital hub which includes established patient groups, and health care professional teams. The hub will supply a safe and secure social platform that provides the capacity for discussion, dissemination of robust information and provision of peer support, signposting of people to this space and to CVUHB, capacity for members to track and record their symptoms and journal their health experience and use the symptom checker (including download of the symptom checker report).
