Lot 1: Integrated Care Programme- The service provides person centred support for people aged 50+ who are at risk of an unplanned hospital admission, a high frailty score or any number of long-term conditions. The approximate lifetime value of the contract is £248850.75 however the final contract value is still to be finalised. Contract Start Date: 01/04/2024 Contract End Date: 31/03/2025 The ICB are intending to award a contract to an existing provider following direct award process C.
