Community Dementia Support service to deliver specialist information, advice, guidance and support with and in local communities, working with health and social care in a way that supports person-centred care, sustainability and better outcomes for people living with dementia, their carers and families, thus ensuring that people living with and affected by dementia receive the right support at the right time in the right place. Lot 1: Core Dementia Services Community Dementia Support service to deliver specialist information, advice, guidance and support with and in local communities, working with health and social care in a way that supports person-centred care, sustainability and better outcomes for people living with dementia, their carers and families, thus ensuring that people living with and affected by dementia receive the right support at the right time in the right place. Lot 2: Non-Core Dementia Services Allowing for ECC or partners within the Essex County Council administrative footprint to “call off” provision for agreed ICS, Alliance or Health & Care Partnership needs-led interventions, projects or schemes allowing additional investment
