Tender description

The service supports significant numbers of discharges each week-20 at minimum which means people are more likely to go home when they are medically fit to do so and there will then be limited delayed hospital stays which will reduce costs and negative experiences for individuals who have prolonged hospital stays. The packages of reablement will improve people's independence and wellbeing allowing them to live as independently as possible at home, reducing their need to access statutory services Lot 1: This is a Provider Selection Regime (PSR) confirmation of intention to award notice.
