Tender description
The service will operate under an APMS contract and provide core General Medical Services with an emphasis on Neighbourhood Integration and development to improve outcomes for patients registered with the practice and with the wider Primary Care Network. This new APMS contract will provide Core General Practice Medical services and in addition the provider will need to actively contribute to neighbourhood development in line with the Next Steps For Integrating Primary Care Fuller Stocktake Report. They must deliver the Network Contract Direct Enhanced Services (DES), the Modern General Practice Model and the NHS delivery Plan for Recovering Access to Primary Care.
