Tender description
Prior Information Notice to provide advance notification of the forthcoming contract opportunity for Argyll & Bute Health and Social Care Partnership (HSCPs). The A&B HSCP are looking to continue providing the Community Link Worker service within Argyll and Bute. Lot 1: The scope of the services covered under the contract will include but not limited to, Services to support people to live well through strengthening connections between community resources and primary care and developing pathways to community and third sector services and activities. The Community Link Worker will provide a person-centred service that is responsive to the needs and interests of patients registered with GP Practices in socio-economically deprived and remote/rural areas of Argyll & Bute.
