Tender description

The Camden Adult Pathway Partnership is a small clinical team that provides nurse-led outreach to 15 supported accommodation sites across Camden, known collectively as the Adult Pathway. The service provides a health outreach model with a multi-disciplinary approach for single homeless people with complex needs, including ex-rough sleepers. Lot 1: Following a direct award process, in accordance with Direct Award process C under the Provider Selection Regime (PSR), the Council are proposing to award a contract to University College London Hospitals NHS Foundation Trust (UCLH) for a period of 12 months from 1st April 2024 until 31st March 2025 for a total cost of £244,110.
