Pre-market engagement Market Research information has been garnered from the previous Discovery and Alpha phases plus the pilots detailed within the specification. Work done so far Discovery phase Alpha Phase Cornwall Evaluation Southwark Pilot For a more detailed breakdown on these please refer to the published specification Which phase the project is in Beta Existing team DHSC has overall responsibility for the digital NHS Health Check, consequently the senior responsible officer and service owner are DHSC team members. As the service will sit within the NHSE architecture and will be managed by NHSE, we have developed joint governance arrangements. The Supplier will be expected to work in partnership with DHSC and NHSE, forming blended service delivery teams that adhere to the standards and processes required within NHSE. Based on the work undertaken during Alpha, we anticipate requiring approximately three multi-disciplinary squads to deliver cross-cutting activity and core digital capabilities for the Build phase and two multi-disciplinary squads to deliver the private Beta. This delivery team will work alongside and report to the joint Delivery Board. As part of our approach to collaborative working, teams will be encouraged to work out of the DHSC London offices where possible alongside the multi-disciplinary teams. The digital delivery team will be expected to work in a multidisciplinary team, including expertise across the following broad categories: 1. NHS Health Check policy/programme specialists 2. Subject Matter Experts 3. NHSE Transformation team 4. Digital experts from across DHSC and NHSE 5. Behavioural scientists The Department currently estimates that two to three delivery squads will be required to meet the Build stage requirements and a further one to two squads to deliver the Private Beta phase (including any extension). The tender evaluation and awarded contract will be based on the bidders’ ability to provide the full five squads however some internal resource may be used to reduce the size of each squad provided by the supplier. This will be agreed with the supplier as part of each statement of work. Consequently, the programme is not contributing as much as it could to reducing premature CVD mortality, and morbidity as a result of conditions including high blood pressure (hypertension), heart disease, stroke, type 2 diabetes, kidney disease and some types of dementia. The digital NHS Health Check will be a single innovative national digital service which will bolster local authority delivery of the NHS Health Check programme through an additional delivery model, enabling increased overall participation and reducing variation between local authorities. The digital NHS Health Check will meet existing legislative requirements. The check will empower users to take actions to improve their health independently, link to national and local services, and enable general practice to initiate clinical care where appropriate. The digital NHS Health Check will meet the existing legal requirements for the NHS Health Check, which covers the following elements: • Identify eligible population • Invite eligible individuals • Questionnaires and biometrics • Communicate results to the end user and their general practitioner (GP) • Facilitate clinical and self-care follow-up The intention is that the digital NHS Health Check can be self-completed independently by eligible participants and used by providers to facilitate delivery of high-quality in-person checks in community and primary care settings according to local commissioning models. The digital NHS Health Check should provide end-to-end data on patient journeys enabling national and local commissioners and local providers to understand opportunities for improvement. Our ultimate aim is to increase the flexibility and reach of the NHS Health Check through the creation of an innovative, accessible national digital offer, delivered alongside local face-to-face offers, that helps people understand and take action to engage with their cardiovascular health. Between April 2015 and March 2020 (pre-pandemic), around 41% of the eligible population completed a check. Between April 2017 and March 2022, 10,037,340 (63%) eligible people were offered and 4,495,897 (28%) had an NHS Health Check. Data on the current levels of NHS Health Check activity can be found on Fingertips. There is significant variation in the number of NHS Health Check invitations offered and completed between local authorities. Between 2015 and 2020 across different local authorities, the proportion of people completing an NHS Health Check varied from 10% to 80%. The majority of check are conducted in GP surgeries. Currently 89% (124) of LAs reported that GPs or GP Federation delivered at least 3 quarters of the checks in their areas. The vision is to increase the flexibility and reach of the NHS Health Check through the creation of an innovative, accessible national digital service, delivered alongside local face-to-face offerings, that helps people understand and take action to engage with their cardiovascular health. The vision is to increase the flexibility and reach of the NHS Health Check through the creation of an innovative, accessible national digital service, delivered alongside local face-to-face offerings, that helps people understand and take action to engage with their cardiovascular health First user type: The Public Enter more details about this user type: The Local Authorities Functions Regulations state that all people aged 40 – 74 years who do not have any of the following conditions are eligible for the NHS Health Check every 5 years: • coronary heart disease • chronic kidney disease (CKD), which has been classified as stage 3, 4 or 5 within the meaning of the National Institute for Health and Care Excellence (NICE) clinical guideline 182 on CKD • diabetes • hypertension • atrial fibrillation • transient ischaemic attack • hypercholesterolemia – defined as familial hypercholesterolemia • heart failure • peripheral arterial disease • stroke • is currently being prescribed statins for the purpose of lowering cholesterol • people who have previously had an NHS Health Check, or any other check undertaken through the health service in England, and found to have a 20% or higher risk of developing cardiovascular disease over the next ten years. Where someone has a CVD risk of 10-19%, they would not be excluded from recall. This is unless they meet one of the other exclusion criteria, for example, if the individual is being prescribed a statin. The digital NHS Health Check is not intended to replace the in-person check. Instead, it will strengthen local NHS Health Check delivery activity by offering a complimentary alternative to local in-person provision. By making it easier for those who can self-check to do so, we can free up local in-person NHS Health Checks. As the organisations legally responsible for provision of the NHS Health Check, we envision that local authorities will be able to choose how they wish to use a digital health check service in their area, including who they wish to target the digital offer to. The intention is that commissioners and planners will prioritise and improve access to in-person checks to people those who prefer to complete their check face-to-face, those most likely to develop CVD, and underrepresented groups, while a digital offer will ensure that access to the service remains universal for those at lower risk of CVD, or for those willing and able to engage in a digital check.
