Tender description

The Community Care Nurse Specialist Service (CCNSS) is integral to ensuring eligible service users receive appropriate palliative and end of life care. It uses a holistic approach to support the complex needs of the individual, whether they are physical, social, psychological, religious or spiritual that their core team alone cannot. Community Care Specialist Nurses deliver the CCNSS and work as part of multidisciplinary teams (MDTs), providing the service directly to the individual and those important to them and/or supporting others to do so.
