Tender description

The requirement is for clinical services for children and young people (up to the age of 25) living with Type 1 and Type 2 diabetes. This will ensure support and universal access to digital self- management resources, so that children and young people with Type 1 diabetes can access high quality resources, supporting them in managing their condition. We also wish to support care team with out of clinic contacts to support children and their families to manage the condition.
