Community Rehabilitation: Discharge flow research model

Tender description

A quotes process was completed for external support is required for the Community Rehabilitation programme to undertake research and modelling to help local decision makers to understand the impact of making changes in the health and social care system on future post-discharge activity, demand and the supply of services. This will be a part of the Discharge flow analytical model to allow systems to benchmark key measures of capacity, population and demand, against other systems, standardised by populations, and to identify impact of shifting from bedded to home models of care.
