HAC5940 - Independent Living and Community Support (ILCS)

Tender description

The Tower Hamlets ILCS service will offer multiple functions: 1 - Floating support: visiting residents in a setting of their choice to provide face to face support. The service may make use of virtual contact, but this should not replace face to face connections. 2 - Drop in’s: the service will provide weekly drop ins for current or historic service users to attend should they require urgent support. 3 - Facilitating move on: the service will connect with those in supported living applying for move on via the borough general needs housing quota. 4 - They will deliver learning around tenancy sustainment and will agree applications in line with the borough existing process. 5 - Supporting those existing supported living: all those moving on via the boroughs general needs housing pathway will be subject to a minimum of 6 months floating support delivered by ILCS. The service will also fulfil historic housing management functions for 7 properties whereby support was previously attached to the tenancies. 6 - Additional functions: the service is expected to maximise delivery, this may include; presence at local boards, development of peer mentoring programmes, use of volunteers, activity groups, service user coproduction, use of local resources etc. Keywords: Support Services, Floating Support, Mental Health
