Nottinghamshire County Council (the Council) wishes to commission a Supported Hospital Discharge Service to assist people being discharged on Pathway 1 who may have little or no support at home to help them undertake basic but nonetheless vital tasks upon their return from a stay in Hospital. The Council is seeking a single Provider to deliver a Supported Hospital Discharge Service throughout Nottinghamshire County (excluding Nottingham City).
