Tender description

The Council is looking to commission a Provider to deliver a Community Wellness Outreach service within West Berkshire. The contract duration is 18 months and will be let from 2nd January 2024 - 30th June 2025. The contract value is £275k and bids for this work should not exceed that amount. The overall aim of the service is to promote the early identification and management of risk factors for CVD, through increased uptake of the NHS Health Checks Pathway by eligible residents and local workforce disproportionately impacted by cardiovascular disease (CVD) but under-served by the universal NHS Health Checks programme in West Berkshire. There is a wider aim to act to reduce health inequalities and build capacity within communities to promote health and wellbeing, across Berkshire West.
