Tender description

County Durham Adult Wellbeing Services that will encompass the following services: • Wellbeing for Life Service • Macmillan Joining the Dots Services • Cancer Awareness and Health Promotion • Health Squad Service • NHS Health Checks Programme - Wellbeing Advisors The new contract will also be structured to allow the inclusion of a case management system in future (if the decision is taken for DCC to cease providing a system for the service) plus additional grant funding that may become available during the contract term that would be relevant for inclusion within this wider Adult Wellness/Wellbeing Service.
