The aim is to recommission the current services and to seek to prevent, reduce and delay the need for more complex ongoing support and for people to remain “living well” and independent for as long as possible. To do this we are seeking to commission a diverse range of preventative support, from light touch, universally accessible interventions through to more complex interventions, for example individuals who are experiencing or are at risk of the following:</span></span><span><span style="font-size: 11pt;
