Tender description
Leeds City Council, in partnership with Leeds Community Healthcare NHS Trust (LCH), intends to pilot a new Community Health and Wellbeing Service (CHWS) in one area of the city, to support people to live well at home. The service is based on reforming how home care is commissioned and delivered and includes Care Workers undertaking delegated health care tasks. We are looking for a suitably experienced Consultant, to undertake an evaluation of this pilot to inform future commissioning plans and the wider roll out of the service.
