Young Peoples Supported Living/Housing Pathway

Tender description

This service is for the Young People's Supported Living/Housing Pathway that has been developed to meet the support needs of young people leaving care and vulnerable young people facing homelessness. The accommodation pathway is designed to provide move-on from care or home setting into a Pathway offering assessment, mediation, guidance, training and support leading to independent living. The step-down accommodation units comprise of shared houses and self-contained flats where support workers are allocated to each young person throughout their stay. Services within the wider pathway include Assessments and Mediation and Supported Living Schemes for young people (aged 16 - 25) who meet the service threshold. The young people who will access the services within this specification include children in our care, care leavers, unaccompanied asylum-seeking children/ young people (UASC), young people at risk of homelessness, young homeless rough sleepers (including sofa surfers), teenage parents, and young people coming out of secure accommodation.
