Applications must be submitted via the online form - link below and in the application pack. An opportunity for Voluntary, Community, Faith and Social Enterprise (VCFSE) organisations to access grants to support health and social care programmes which include: Carers Breaks: To support the commitment of supporting Carers a Grant Fund has been established which will support Third Sector Organisations to develop and operate ‘Carer Breaks Services’ for the benefit of Carers who provide regular amounts of care for the person they care for. Physical Health and Wellbeing: This funding stream will support the other priority areas defined for Cheshire West Place. Proposals should complement local provision (formal and informal support and services) and work with local services to direct to more specialist support where appropriate. Health Home Support: To provide community led support that enables residents to live independently and remain in their own homes and communities. Applications should be completed and submitted online via Apply for the health and wellbeing grant programme - Declaration - Self (cheshirewestandchester.gov.uk) The deadline for applications is 10am 25th September 2023.
