Feasibility Study to inform the need for and scope of collocated and integrated health and social care services and intermediate care facilities across Wrexham Contract Overview The Integrated Rebalancing Care Fund (IRCF) supports the development of two priority areas: 1. The delivery of collocated, integrated health and social care services 2. Rebalancing the residential Social Care market. This proposal promotes the strategic vision of IRCF, to offer a proactive, coordinated and co-located approach to service provision, focused on the needs of the local population. We are commissioning a feasibility study to inform the need for and scope of collocated and integrated health and social care services and intermediate care facilities across Wrexham. The study will be completed within a 6 month timescale. . CONTRACT PERIOD The contract is for a period of 6 months. The contract is to commence as soon as possible but no later than 31st March 2023.
