1. Core Services to Support Older People; Dementia support, Provision of Information and Advice and Volunteering opportunities. It is recognised that to stay well, live at home and maintain independence older people need access to support with; the opportunity to get involved (including volunteering) and quality information and advice. Dementia can have a significant impact on the lives of many older people and the right support for people living with dementia and their carers leads to greater independence and wellness for longer. 2. Community Support Service Development and maintenance of skills, connection and confidence help people to stay healthy and independent for longer. Connecting to community is an important part of staying well, but once a person starts to feel isolated it can be difficult to reverse without support. Signposting at each stage, befriending and buddying are approaches recognised as supporting people to reconnect to their community and regain the skills and confidence to live healthily at home for longer.
