Tender description

Havering's Reablement service focuses on supporting residents to regain skills and confidence to enable them to remain living independently or with less support in their own home following a hospital admission. The service operates under the 'Home First' model which means their care and support needs are assessed by the provider in their own home as opposed to the acute hospital setting. The Home First model reduces the level of assessment in an acute setting, ensuring no decisions about long term care are made whilst the person is in an acute setting. This includes decisions about residential care. We would like to hear from potential providers interested in delivering a reablement service in Havering, to inform the planning of our procurement process.
